Medical Specimen Transport Requirements: A Lab-Directed Handoff Framework
Preparing a specimen for pickup is not simply a packaging task. It is a handoff between the clinical team, the courier route, and the receiving laboratory. The most useful question before pickup is whether the item is ready under that laboratory’s directions, with the information and handling details needed for the requested test.
Requirements can differ substantially between tests, including the needed container, preservative, identification, and how quickly the laboratory needs to receive the item. One pathology laboratory, for example, distinguishes routine submissions from special biopsies and tests that need different procedures or fixatives. That is why the receiving laboratory’s specimen requirements should guide each shipment rather than a generic transport checklist. This article offers a pre-handoff framework, not a replacement for laboratory-specific instructions.
Start With the Receiving Laboratory’s Test-Specific Instructions
Review the receiving laboratory’s collection, submission, and transport directions before scheduling or releasing a specimen. Those directions are the controlling source for the requested test. They may address the type and size of container, whether a preservative or other medium is needed, how the specimen should be labeled, a temperature condition, and the maximum time before receipt or processing.
These details vary because the laboratory is preparing to perform a particular method on a particular kind of material. A process that is appropriate for one test may be unsuitable for another. For example, a laboratory may give different preparation directions for a routine tissue submission than for a specialized test. That example does not create a rule for every laboratory or every specimen. It shows why the test order and the laboratory’s own directions must be considered together.
Separate the instructions into two groups as you prepare the pickup. First, identify the test-specific details that must be followed exactly: the required container, any required additive or preservative, stated temperature condition, routing destination, and time limit. Second, identify the common handoff checks that support an orderly transfer, such as confirming the request paperwork and a receiving contact.
Do not fill gaps by choosing a familiar container, assuming a storage condition, or applying a workflow used for a previous test. If the order is unusual, the directions conflict, the specimen was collected outside the expected timing window, or a requirement is unclear, resolve the question with the receiving laboratory before the specimen leaves the clinical setting. This is especially important when several specimens or different tests are being sent together, since each item may have its own instructions.
Verify Identification and Request Information Before Pickup
Before handoff, compare the specimen label with the accompanying request or requisition and with the laboratory’s submission directions. The aim is not to impose one universal form. It is to make sure the specimen can be connected accurately to the requested work when it reaches the destination.
Start with the identification elements specified by the receiving laboratory. Depending on its workflow, those may include the patient’s identifying information and the specimen source or collection site. A pathology laboratory’s published requirements, for instance, call for patient identification and the specimen source, along with pertinent information on the request slip. Other laboratories and tests may specify a different set of fields, so use the instructions for the destination rather than copying that example.
Then check the requisition or request for the information needed to perform and route the ordered test. Review the requested test, specimen source when applicable, collection information when requested, ordering clinician or facility details, and any other fields the receiving laboratory identifies. If the test is a send-out or coordinated submission, verify the destination and any laboratory reference number or routing information already assigned.
A useful check is to ask whether a staff member receiving only the label and request could tell that they belong together. If a label and request disagree, the source is missing, the test cannot be identified, or the paperwork is incomplete under the laboratory’s process, pause before release and correct or clarify the discrepancy through the appropriate clinical or laboratory channel.
This check is practical, not a guarantee. A courier can transport an item that is handed over, but clinical staff and the receiving laboratory remain the appropriate sources for questions about collection, test ordering, documentation, and acceptance. Completing the comparison before pickup helps prevent an avoidable interruption after the item is already in transit.
Prepare Closed, Separated Transport Packaging
Once the laboratory-directed preparation is complete, prepare the specimen for a clean, contained handoff. Make sure the primary specimen container is closed securely and inspect its exterior for visible contamination or leakage. Then place it in the closed transport packaging specified by the laboratory or facility procedure.
Paperwork deserves its own protection. Keeping the request or requisition physically separate from the specimen helps keep documents readable and limits the chance that a leak affects them. UCSF Clinical Laboratories’ transport guidance illustrates this approach: tightly capped containers go in a biohazard bag, while accompanying paperwork is placed in the bag’s external pocket rather than with the specimen. Its instructions also call for clean container exteriors and secure lids before transport.
Treat that as a clear example of an institutional packaging process, not as a universal package design. Your facility or receiving laboratory may provide a different bag, sleeve, transport box, absorbent material, label, or closure procedure. Use the packaging system and handling directions that apply to the test and destination. If the laboratory calls for a biohazard-marked transport container or another specific outer package, confirm that the selected packaging meets that instruction before pickup.
Avoid placing loose papers where they can contact the specimen. Avoid sending a container with a questionable lid or a visibly soiled exterior. If an item appears to be leaking, damaged, incorrectly contained, or unable to be closed as required, follow the relevant facility and laboratory procedure before arranging the handoff. Do not rely on a courier pickup to correct a packaging issue.
For more than one patient or more than one specimen, keep each item and its corresponding documentation organized according to the facility’s process. Clear separation reduces the risk of mixing labels, requests, or handling directions during a busy pickup.
Communicate the Handoff Details That Affect Timely Receipt
A time-sensitive pickup needs a clear operational handoff as well as correct preparation. Give the courier the pickup location, the destination, the receiving department or laboratory contact, and the time the specimen will be ready. If there is a stated delivery window or laboratory cutoff, communicate it as provided by the laboratory rather than translating it into an assumed arrival promise.
Pass along any laboratory-provided handling, routing, or timing instruction in a form the courier can use. This might include the destination entry point, a reference or order number, a request to notify a receiving contact, or a stated condition for transport. The instruction itself comes from the laboratory; sharing it does not establish that every courier or route can meet every requested condition. Confirm operational details before handoff when the instruction is unusual or the timing is tight.
For multiple specimens, coordinated deliveries, or a submission type the laboratory asks to arrange in advance, contact the receiving laboratory before pickup. Confirm where it should be delivered, who should receive it, and whether there is a particular handoff procedure. This prevents a specimen from reaching the right campus or building but the wrong department or receiving point.
At handoff, staff can make a concise, practical confirmation: laboratory directions were reviewed; the specimen label and request were matched; the container is closed and prepared under the applicable packaging process; the destination and receiving contact are confirmed; and any time-sensitive or handling instructions have been communicated. These checks support a clearer transfer, but they do not guarantee specimen integrity, laboratory acceptance, safety, or regulatory compliance.
